HOJ Deaf Club

Annually HOJ Deaf Club recognizes and and honors the accomplishments of deaf and hard of hearing individuals who in their own way support the ideals of the National Action Network.  The banquet takes place on the first Saturday in February.

1. Name of nominee and contact information.  (If selected we will contact them)
2. Name of agency or organization where nominee currently works or volunteers.   Please give their title.
3. Length of time nominee has been involved working for equal rights in the deaf and hard of hearing community.
4. As a result of the nominee’s involvement, what changes have occurred in the deaf and hard of hearing community?  
5. Please give specific examples of how nominee has advocated for deaf and hard of hearing rights.
